Summary:

Getting the right diagnosis is a key aspect of health care - it provides an explanation of a patient's health problem and informs subsequent health care decisions. The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001) finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendations of Improving Diagnosis in Health Care contribute to the growing momentum for change in this crucial area of health care quality and safety.

Summary:

In the 25 years since the first edition of Comprehensive Gynecology, many scientific advances have occurred in medical practice. The first four editions were largely the work of the original four editors: Drs. William Droegemueller, Arthur L. Herbst, Daniel R. Mishell, Jr., and Morton A. Stenchever...With the staggering volume of medical literature published and the complexities of the gynecologic subspecialties, we have collaborated with additional experts for the sixth edition. We've "examined disease" and added a new chapter on the interaction of medical diseases and female physiology. We've "investigated discord" with new authors to completely rewrite the emotional and psychological issues in gynecology and the legal issues for obstetrician-gynecologists. Other chapters have delved into the controversies in breast cancer screening, vitamin D use, the ongoing debates in hormone therapy, and vaginal mesh use for pelvic organ prolapse surgery. Summary:

Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Within each, the chapters are structured into sections containing pedagogic features such as boxes, highlights and key points for the first time. It contains everything the clinician needs to practice the art of obstetrics and gynaecology and sufficient information to help sub-specialists develop their specific interests. This text is recommended reading for the Royal College of Obstetrics and Gynaecology membership examination. This seventh edition contains 21 new chapters: Anatomy of the Pelvis and Reproductive Tract Placenta and Fetal Membranes Antenatal Care Anaesthesia and Analgesia Recurrent Miscarriage Ectopic Pregnancy Trophoblastic Disease Genetics and Prenatal Diagnosis Drugs and Pregnancy Obstetric Emergencies Prolonged Pregnancy Renal Disease Termination of Pregnancy Imaging in Gynaecology PMS Assisted Reproduction Hysteroscopy and Laparoscopy Sexual Dysfunction Psychological Aspects of Pregnancy Domestic Violence and Sexual Assault - now on MRCOG curriculum Ethical Dilemmas This seventh edition has been significantly updated with a wide range of internationally renowned contributors who are all experts in their field, bringing this book to the cutting edge of knowledge in obstetrics and gynaecology. "It was my favourite textbook when I was working for my examinations and I am still using it. I can recommend this edition to current trainees wholeheartedly. It is certainly worth buying." Summary:

This book highlights minimum standards relating to the management of different conditions in the practice of Obstetrics and Gynaecology. The editors explore clinical governance issues, common causes of as well as ways to avoid litigation. The UK is experiencing a dramatic increase in medico-legal claims. The 4 main reasons for litigation are: accountability, the need for an explanation, concern with standards of care and compensation. However the decision to take legal action is determined not only by the original injury, but failure to provide information, an explanation and an apology. Insensitive handling of an injury and poor communication after the original incident increases the risk of litigation and erodes the patient-doctor relationship. Doctors almost never deliberately cause harm to patients, however increasingly claims are being defended successfully. This book is invaluable to clinicians and lawyers alike and raises awareness of how to avoid facing clinical negligence claims in our day to day practice.

Summary:

"In this book, we tell the story of burnout of health care professionals. Many believe burnout to be the result of individual weakness when, in fact, burnout is primarily the result of health care systems that take emotionally healthy, altruistic people and methodically squeeze the vitality and passion out of them. Burned-out professionals are exhausted, jaded, demoralized, and isolated, and they have lost their sense of meaning and purpose. Frequently, these individuals are shamed and blamed by leaders who suggest they should sleep longer, meditate, and become more resilient even as they expect them to work harder, see more patients, embrace rapidly changing technology, stay abreast of new medical advances, and provide quality health care"--Provided by publisher.

Summary:

The thirteen case studies contained in this publication were commissioned by the research node of the Knowledge Network on Priority Public Health Conditions (PPHC-KN), a WHO-based interdepartmental working group associated with the WHO Commission on Social Determinants of Health. The publication is a joint product of the Department of Ethics, Equity, Trade and Human Rights (ETH), Special Programme for Research and Training in Tropical Diseases (TDR), Special Programme of Research, Development and Research Training in Human Reproduction (HRP), and Alliance for Health Policy and Systems Research (AHPSR). The case studies describe a wealth of experiences with implementing public health programmes that intend to address social determinants and to have a great impact on health equity. They also document the real-life challenges in implementing such programmes, including the challenges in scaling up, managing policy changes, managing intersectoral processes, adjusting design and ensuring sustainability. This publication complements the previous publication by the Department of Ethics, Equity, Trade and Human Rights entitled Equity, social determinants and public health programmes, which analysed social determinants and health equity issues in 13 public health programmes, and identified possible entry points for interventions to address those social determinants and inequities at the levels of socioeconomic context, exposure, vulnerability, health outcomes and health consequences.

Summary:

Decades of research have demonstrated that the parent-child dyad and the environment of the familyâ€"which includes all primary caregiversâ€"are at the foundation of children's well- being and healthy development. From birth, children are learning and rely on parents and the other caregivers in their lives to protect and care for them. The impact of parents may never be greater than during the earliest years of life, when a child's brain is rapidly developing and when nearly all of her or his experiences are created and shaped by parents and the family environment. Parents help children build and refine their knowledge and skills, charting a trajectory for their health and well-being during childhood and beyond. The experience of parenting also impacts parents themselves. For instance, parenting can enrich and give focus to parents' lives; generate stress or calm; and create any number of emotions, including feelings of happiness, sadness, fulfillment, and anger. Parenting of young children today takes place in the context of significant ongoing developments. These include: a rapidly growing body of science on early childhood, increases in funding for programs and services for families, changing demographics of the U.S. population, and greater diversity of family structure. Additionally, parenting is increasingly being shaped by technology and increased access to information about parenting. Parenting Matters identifies parenting knowledge, attitudes, and practices associated with positive developmental outcomes in children ages 0-8; universal/preventive and targeted strategies used in a variety of settings that have been effective with parents of young children and that support the identified knowledge, attitudes, and practices; and barriers to and facilitators for parents' use of practices that lead to healthy child outcomes as well as their participation in effective programs and services. This report makes recommendations directed at an array of stakeholders, for promoting the wide-scale adoption of effective programs and services for parents and on areas that warrant further research to inform policy and practice. It is meant to serve as a roadmap for the future of parenting policy, research, and practice in the United States.
