Since its discovery as the cause of infectious mononucleosis in 1964, the Epstein-Barr virus (EBV) has been etiologically implicated in an increasing number of human diseases. Generally considered the first human oncogenic virus because of a number of studies linking it with Burkitt's lymphoma and nasopharyngeal carcinoma (NPC), as well as its documented oncogenicity in nonhuman primates, EBVhas served as a model for identifying subsequent candidate oncogenic viruses and the stimulus for Evans' revision of the Henle-Koch postulates to accommodate the problems in proving viral oncogenicity in humans. Research on the role of EBV in human cancer was particularly en hanced (a) by the pioneering work of Werner and Gertrude Henle, and (b) by the coordinated efforts of the Special Virus Leukemia Program and its successors, the Special Virus Cancer Program and the Virus Cancer Program of the National Cancer Institute (NCI). Initiated by Dr. Frank J. Rauscher, who subsequently became director of the Ncr and is now Vice-President of the American Cancer Society, and expanded by Dr. John B. Moloney, whose contributions to cancer research were honored at this Second International Symposium on EBV and Associated Malignant Diseases, these NCI contract-sup ported programs brought together investigators from all over the world to participate in a joint effort to unravel the mystery of EBV behavior and pathogenicity. Prior to Banting and Best, chnicians were unaware of the ravages of diabetes, patients simply wasted away and died. Following the purifica tion of insulin, clinicians now had to deal with diabetic retinopathy, diabetic neph ropathy and all the other complications of long-term diabetes. A little over 50 years ago, the first successful human kidney transplant was performed in Boston. The first 30 years of the experience had successes when compared to the alternative but were a constant struggle to get even 50% of the grafts from deceased donors to survive more than a year. However, the science continued to advance knowledge of the immune response. With this came more and increasingly powerful tools for the clinician. Suddenly, success rates of 80-90% at one year were attainable. With this success came new problems, new complications and clinicians now had to worry about the long-term consequences of their therapy as patients were surviving with functional grafts for extended periods. A particular infectious complication evolved with the application of ever more powerful immunosuppressant drugs. Astute clinicians noted that occasionally cellular rejections seemed to get worse with steroids. Despite their best efforts and the use of powerful drugs, patients lost their grafts to overwhelming interstitial infiltrates not seen before.

In recent years, papillomaviruses in general and human papillo maviruses in particular have been recognized as possible agents of important diseases, including some forms of human cancer. The purpose of this book is to present a concise panorama of the pre sent status of knowledge of this topic. This knowledge is as impor tant to molecular biologists and virologists as it is to clinicians and pathologists. To bridge the gap among these diverse groups of investigators, we conceived of a book covering a broad spectrum of the basic scientific, clinical, and pathological aspects of diseases associated with papillomaviruses. Although the principal thrust of this book is directed at human papillomaviruses, fundamental knowledge of animal viruses is essential to the current understand ing of the molecular mechanisms of cell transformation. For this reason, a chapter on animal viruses has also been included. Some of the experimental work having to do with the elucidation of transformation and other aspects of interaction between the virus and the cell cannot be based on human papillomaviruses because of a lack of suitable experimental models. Hence, some of the chapters dealing with fundamental aspects of viral molecular biol ogy are based on animal models. We were very fortunate in having persuaded a number of distin guished colleagues to contribute to this work.

The contents of this volume are based upon the proceedings of a Sympo sium entitled "Infectious Diseases of the Central Nervous System" held in Phoenix, Arizona, and sponsored by the Barrow Neurological Institute and Foundation during its Ninth Annual Symposium. The purpose of the Symposium was to bring together knowledgeable experts in this field to review information that is available and to enhance our knowledge of new developments in the field of infectious diseases in the central nervous system. Because the subject could not be covered in its entirety by this volume, we have placed particular emphasis on recent developments and new information. The volume includes a remarkably fresh and interesting discussion of viral diseases as they affect the nervous system, including conventional and unconventional virus agents, and in addition, discussions of path ophysiology and epidemiology and postinfectious diseases of the nervous system. A similar approach is taken to the treatment of bacterial infection. Discussions of pathophysiology are intertwined with discussions of diag nostic techniques, prevention techniques and treatment of bacterial in fections. Additional surgical problems are discussed regarding prevention and management of peri operative infection and brain abscess. Special consid eration was given to coccidioidomycosis which is prevalent in the western states; also, there is a discussion of parasitic infections.
